Adults and their caregivers, with needs related to physical disability, aging and/or mental health, who live in Durham Region have equal access within each local municipality to the range of services offered by Community Care. We believe: - a person's independence is best maintained by a stong personal and/or community support system; - all people should be approached with empathy, dignity and respect; - people have a right to confidentiality and privacy; - people have the right to make their own informed decisions; - in a service model that focuses on well-being, independence and abilities within a continuum of care, and works collaboratively with the other community agencies; - a person's needs are best met through the decentralized provision of community services; - quality care is assured by service standards being defined and practiced; - volunteers and staff play a significant role in the efficient and effective delivery of community support services; - volunteers and staff must be appropriately supported; in the value of community input to ensure that our services are responsive and of high quality; - in avoiding duplication of services; - in the not-for-profit delivery of community health services; - communities are strengthened by people helping people Durham Region Community Care is a non-profit registered charitable organization, established in 1977, and incorporated in 1983. Programs: Community Care provides three programs, each having a range of support services: the Home Support, Respite and the COPE Mental Health Programs. Eligibility criteria apply to all services. A nominal fee applies to Respite and some Home Support services. Services are provided subject to the availability of volunteers, helpers and budget limitations. Services are not restricted to office hours (8:30 a.m. - 4:30 p.m. weekdays).
